Cryolipolysis/ Fat Freezing: Name:____________________ Date of Birth:______________
Coldness stimulates the blood circulation and fat cells get eliminated at about 0° Celsius. The surface temperature in cryolipolysis is about -8 until -12° Celsius. The consequence is an desiccation of the fat cells. The full effect is reached after 60-90 days. During this time lymph drainage / Ultratone, regular exercise and a carbohydrate-reduced diet are absolutely necessary to achieve maximal effects and to prevent the free fat to get stored in other (untreated) areas of the body. For life this treatment can just be conducted maximally 3 times per body region, earliest 3 months after the initial treatment. There should be at least 5 days between the cryolipolysis and an ultrasound/ cavitation treatment. Contraindications:
• Pacemaker, heart valve replacement, severe heart diseases
• Autoimmune diseases such as: cryoglobulinemia,
paroxysmal cold hemoglobinuria, cold urticaria
• Metal implants in the treatment zone
• Other metals due to accident or surgery
• Impairment of skin feel in the treatment zone (eg, after surgery)
• Keloid / hypertrophic scar in the region
• Anticoagulant drugs (e.g., warfarin, heparin, low-dosage aspirin)
Did you have any cryolipolsysis, invasive or similar beauty treatments before?
If yes, which area and what kind of treatment?____________________________________ Pre-existing conditions:______________________________________________________ Medication:_______________________________________________________________ Possible side effects: A cold or hot feeling, skin redness or possibly a hematoma (bruise) can occur in the treated area. I have read the questionnaire and have it explained and I agree with the possible side effects. Ultrasound Cavitation: Name:____________________ Date of Birth:______________
The ultrasound cavitation activates the cell water which leads to pulsation of the fat cell. The fat cell membrane inflates and collapses - the fat cell implodes. The fatty acids are released rapidly (within 24 hours) and the lymphatic vessels, the liver and the kidneys are challenged to drain them. The depletion process takes 3 days. It is important to drink a lot during this time and to reduce the carbohydrate intake to prevent the transport of the released fat to other regions of the body. Lymphatic drainages, exercise and Ultratone improve the effect of the treatment. Contraindications:
• Anticoagulant drugs (e.g., warfarin, heparin, low-dosage aspirin .)
Did you have any invasive or similar beauty treatments before?
If yes, which area and what kind of treatment?____________________________________ Pre-existing conditions:______________________________________________________ Medication:_______________________________________________________________ Possible side effects: During treatment a loud buzzing noise comes up. Therefore we ask you to inform us about ear diseases or tinnitus in advance. I have read the questionnaire and have it explained and I agree with the possible side effects. Radio Frequency Therapy:
Name:____________________ Date of Birth:_______________
The radiofrequency is a heat therapy. Temperatures can be reached of 60°- 65° Celsius in the connective tissue, and 38°- 41° Celsius to the skin surface. The temperature is produced in the deep tissue and radiated to the skin surface. Thus, the blood flow is increased and stimulates the production of collagen fibers. The so-called young collagen is produced only in the first 25 years of your life from the body, then drops the production. At 50° Celsius the collagen breaks down and the young collagen production is stimulated. The young collagen production and their installation requires 2-3 weeks until they are stable. Vitamin C is very helpful at this time. The radio frequency therapy therefore should be performed at maximum every 10 days in the body, in the face a maximum of every 3 weeks. A booster is recommended every 3-4 months. Contraindications:
• Implants, esp. in the treatment area
• Dental implants (locally pain can come up)
• Metal in the body due to accident or surgery
• Hyaluronic acid injections (1-2 weeks out)
• Thyroid disease (Hashimoto's thyroiditis) - avoiding the thyroid area
Did you have any invasive or similar beauty treatments before?
If yes, which area and what kind of treatment?___________________________________ Pre-existing conditions:_____________________________________________________ Medication:______________________________________________________________ Possible side effects: The heat can lead to pain and distension. Please tell right away if it gets too hot, otherwise combustions may occur! Further a temporary skin irritation or redness can come up. I have read the questionnaire and have it explained and I agree with the possible side effects. I understand that the Health Center will not take any responsibility if I give incomplete or false information. Date:________________
